申込者の誓約

私は大会要綱における出場資格、エントリー規定及び注意事項を良く理解し、同意した上で出場を申し込みます。一度支払った大会参加費はいかなる理由があっても返金されないことを了承します。試合においては大会ルールを守り全力を尽くして正々堂々と戦うことを誓います。大会運営のための主催者からの指示には速やかに従います。私は大会中の事故により負傷・死亡する危険がある事と、怪我による後遺症を負う可能性があることを了承した上で出場を申し込みます。またそれらの事故・負傷・死亡・後遺症に対しては、誰にも責任の所在を問うものではなく、主催者及び関係者に対して一切の保障を請求しないことを誓約し、出場を申し込みます。私は、本大会に関するあらゆる著作権・肖像権およびそれらに付帯する権利は、すべて主催者に帰属することを了承します。またパンフレット、場内掲示、トーナメント表及び公式結果の掲載等、本大会実施に必要な範囲において、集められた個人情報を主催者が利用することを了承します。

WAIVER AND RELEASE OF LIABILITY AND AGREEMENT TO PARTICIPATE

I agree to the rules set forth by a competition host and to compete with the qualities of good sportsmanship.I hereby agree to participate knowing there is a risk for an injury, death and post-traumatic syndrome that happens during or after the competition. I also agree not to sue the competition host, or any related organizations for an accident, injury, death, and post-traumatic syndrome that happened during or after the competition. I have read and understood the above statement and I, hereby, agree to participate in the tournament at my own risk. I give up any rights to the tournament host for copyrights related to the competition. I also agree to the public display of any personal information through brochures, official results, etc.
