TF Camp Application form

  • このイベントには中学生以上の方が参加できます。 This event is open to junior high school students and older.
  • 所属しているトライフォース支部名を記入してください。例：『浅草橋』 Please enter the name of the Triforce branch to which you belong. Example: “Asakusabashi”.
    ※参加費は参加区分に関わらず一律2,000円となります。 *The participation fee is a flat rate of 2,000 yen regardless of the participation category.
  • ※当日の入場受付時にお支払いください。 *Please pay at the admission desk on the day of the event.

  • 申込者の誓約
    私はイベント中の事故により負傷・死亡する危険がある事と、怪我による後遺症を負う可能性があることを了承した上で参加を申し込みます。またそれらの事故・負傷・死亡・後遺症に対しては、誰にも責任の所在を問うものではなく、主催者及び関係者に対して一切の保障を請求しないことを誓約し、参加を申し込みます。私は、本イベントに関するあらゆる著作権・肖像権およびそれらに付帯する権利は、すべて主催者に帰属することを了承します。

    WAIVER AND RELEASE OF LIABILITY AND AGREEMENT TO PARTICIPATE
    I hereby agree to participate knowing there is a risk for an injury, death and post-traumatic syndrome that happens during or after the event. I also agree not to sue the event host, or any related organizations for an accident, injury, death, and post-traumatic syndrome that happened during or after the event. I have read and understood the above statement and I, hereby, agree to participate in the event at my own risk. I give up any rights to the seminar host for copyrights related to the event. I also agree to the public display of any personal information through official website, etc.
  • 上記の誓約内容に同意し、セミナーへの参加を申込みます。 I approve the contract above and participate in the competition.
    申込者が未成年であった場合、親権者の同意が必要となります。In the case that the applicant is a minor, participation requires the consent of a legal guardian.

    申込者の親権者である私は、上記の誓約内容を了承した上で、申込者の本イベントへの出場を申込みます。 I, as the legal guardian of the applicant, have read and agreed to the above pledge, and I hereby submit this application for the applicant’s participation in the event.
